...whenever I've gotten a sacred object blessed, it seems the priest just says an impromptu blessing. If a priest blesses a medal, scapular, rosary, salt, etc., and merely says something like: "God, we ask you to bless this object. Amen." - Is the object a valid sacramental? Is the blessing valid?

The blessing is in the Sign of the Cross, not in the words. If he made the Sign, it's a valid blessing.
